Aviation Medical Requirements & Documentation
Please download and carefully read the Aviation Medical requirements. Print out the SACAA Medical Application Form and the X2 medical questionnaires.
Complete and return to us BEFORE we can make your appointment.
You will need to submit these completed docs to us via email:
1. SACAA Application Form for a medical certificate
2. Aviation Medical Mental Screening Questionnaire (Form CA 67-08)
3. SACAA Client Consent Form (Form CA67-39)
4. Any other relevant medical reports you may already have
Please take note of ALL requirements and procedures outlined in the downloadable document, as your medical certificate cannot be issued if you do not meet all the requirements.
Only once we have received and reviewed these documents, can we contact you to make an appointment to come in to our rooms for your medical.
A Chest X-Ray will be required for your initial medical only. We will send you a referral for the XRAY once we have received and reviewed your completed documentation.
